DI application documents and additional information [download forms below] The dietetic practice group, Dietetics Educators of Practitioners (DEP), has updated the standardized supervised practice program application form. Feedback from DPD, DI, and CP directors at DEP Area meetings was used to update the form.  From its conception, this form has been designed to reduce the variation in paperwork necessary for students to apply to supervised practice programs.  Given the variety in supervised practice programs, it will not likely meet every program's needs.  Program directors can continue to elect not to use it in their application process.  DPD program directors are encouraged to alert students to these variations in application procedures.  DI and CP Program directors who previously used the 1998 version of the standardized application form must clearly indicate and provide promotional materials about whether the 1998 or the 2004 version of the forms are required by the program and provide access to the version they expect their applicants to use. ADA has discontinued distribution of the 1998 version of the form as a disk through the catalog.  Some students applying to supervised practice programs for November 2004 and April 2005 may have already received application materials.  Program directors will need to keep this in mind if they plan to transition to the new forms this year.
